- Medical Record Review: A physician will review your medical records and determine if the practice is a good fit to meet your clinical needs based on your current pregnancy and medical history. We also review whether the practice can accommodate additional transfer patients based on volume of current patients expected to deliver each month. The records review process is in place to ensure the practice can meet your needs before you transfer care. This review process typically takes 3 business days, but it could be up to 10 business days from when your records are received depending on the volume of records. We will not be able to see you for any appointments until your medical records have been reviewed and you have been formally accepted into the practice.
- Schedule your first visit: After your records have been reviewed and you have been formally accepted to the practice, we will contact you and assist you in scheduling your first appointment.
Transfer Gynecology (Non-Pregnant) Patients
If you have medical records or recent lab results, please feel free to bring with you to your appointment so your provider can review, and we can scan into your chart. If you do not have your historic records, your provider may ask you complete a medical release form so that we may request any applicable records from outside practices and healthcare systems.
If your visit is following up from a recent hospitalization, please bring any hospital records you have so your provider can review with you at your visit and give you the best care.
