Scheduling an Appointment
1. A referral will be required from your medical provider, indicating requested services. This is needed to schedule you for the appropriate appointment type and to ensure adequate allotted time to complete evaluation of your and your baby’s health.
2. Call our office at 771-200-1920. You will be provided with information on completing your initial registration information. You will also need to fill out the New Patient Medical History form and return it to our office.
If you are being referred for diabetes management, please fill out the glucose log below, before arriving to your appointment. Please remember that if you are utilizing a continuous glucose monitor (CGM), you will need to check the readout of your glucose in the CGM app and record the value at the appropriate time intervals (fasting, and 2 hours after each meal).
3. Once we have the referral and your registration information, you will be contacted by our front desk staff to set up an appointment

Genetic counseling services
- Genetic counseling consultations are covered by many, but not all, insurance plans. To check coverage, you may contact your insurance provider and reference the CPT (procedural) code 96041 for genetic consultation. Please note that pre-authorization or insurance approval for genetic counseling does not guarantee that the consultation cost will be covered.
- Meeting with a genetic counselor is required before diagnostic procedures (CVS, amniocentesis). If you receive a bill for the consultation, you may qualify for a discount as a self-pay patient.
- If your insurance provider requests medical documentation to support the necessity of genetic counseling or testing, please reach out to our genetic counselor at 771-200-1920 for assistance.
- To help make your visit go smoothly, we ask that you arrive 15 minutes before your scheduled appointment time. This allows us to complete the check-in process and ensure that everything is ready for your evaluation (such as an ultrasound).
- If you’re running late
- We understand that unexpected things happen, and we appreciate your efforts to notify us at least 24 hours in advance if you need to reschedule.
- If you arrive more than 15 minutes late, we may need to reschedule your appointment, as there won’t be enough time to finish the evaluation. For instance, if your ultrasound is scheduled for 30 minutes and you arrive 15 minutes late, we may not have enough time to complete the exam in the remaining 15 minutes of your allotted appointment time.
- While we’ll try our best to accommodate same-day rescheduling, availability might mean a longer wait or rescheduling for another day. This policy helps us provide the best care and allow enough time to assess both your health and the health of your baby. Thank you for your understanding!
- If we’re running late…We make every effort to be on time. However, due to the nature of high-risk pregnancies, sometimes unpredictable situations may arise. If this occurs, your appointments may be delayed due to an urgent situation involving another patient. Please be assured that the MFM physician will take the necessary time to thoroughly care for you and your baby. We truly appreciate your patience and understanding, and we thank you for trusting us with your care.
