Step 2: Book your first visit
Click the button to book online or call our practice at (202) 481-2050 to book your first visit.
*If a patient needs language assistance services for a language other than English or needs interpreter assistance for sign language due to the patient being deaf or hard of hearing, the patient must contact the physician practice directly 24 hours before their appointment time so that the physician practice may have the appropriate translator/interpreter available.
Step 3: Prepare for Your Visit
During your first visit, we will review your health history and discuss any questions you have about your health.
To help us prepare for your visit and streamline your experience, please check in and complete your health history in our Patient Portal before you arrive.
Click this link and follow the signup instructions to create your Patient Portal account. Once you’ve completed the online registration, this is where you’ll log in to access your health information.
If you are a new patient, click this link to download our records release form, fill it out, and send it to your current provider to return to our office. This will authorize your provider to release your medical records to your Liv provider.
Quick, simple, secure mobile check-in
We offer you the ability to complete the check-in process on your phone prior to your appointment. We recommend you complete this process on your cell phone so you can upload photos of your ID and insurance card.
How does it work?
72 hours before your appointment, you will receive a text message and/or email prompting you to complete your digital check-in process. It’s important that you complete this in its entirety prior to arrival to ensure a contactless check-in experience.
- Text messages will contain a link that will automatically take you to your mobile check-in
- Emails will contain your appointment information and a button that reads ‘Click here to start’
Once you have opened your mobile check-in, you will be prompted to verify your identity by answering a few security questions.
- This ensures that all information you enter is private and secure
After you begin your mobile check-in, you will be presented with a series of questions related to your health. You can also sign office policies and consents, if needed, and make a payment if a balance or copay is due.
Put some thought into your individual health goals so you’re ready to talk through them with your provider. At Liv, we don’t believe in a “one-size-fits-all” healthcare approach and are dedicated to helping you set and achieve your personalized goals.
Please do your best to be on time for your appointment. If you are going to be more than 15 minutes late, we ask that you contact our team to reschedule your appointment at another time.

Telehealth Appointments
For telehealth appointments, you will receive a text message 20 minutes prior to your appointment prompting you to check in and enter the virtual waiting room.
Once you open the link, you will be prompted to confirm information and will be provided with the link to your Zoom meeting with your provider.
Make sure you provide us with an up-to-date cell phone number and email address to streamline your arrival at the doctor’s office with Mobile Check-in.
COVID-19 precautions
- As a part of your check-in process, we will be screening for COVID-19 symptoms, previous positive COVID-19 test results, and temperature.
- We are cleaning all common areas and exam rooms based on CDC guidelines.
- Anyone who enters the Liv space will be required to wear a mask. Please refer to this guide by the CDC for acceptable mask types.
- Our team members have full access to receiving the COVID-19 vaccine.
- We will ensure social distancing in the waiting area and will text patients who opt to wait outside when they’re ready to be seen.
- Virtual appointments are available for certain appointment types.

Taking just a few minutes to register our patient portal will give you access to valuable information and services provided in a secure and confidential manner. Once registered and logged on you will be able to:
- Book new appointments and manage upcoming appointments
- View lab and test results
- Pay bills and view billing history
- Request prescription refills
- Complete and update medical forms
- Update your profile and contact information
- Contact your doctor about non-urgent medical questions
